Monday Night Youth Night for Grades 1-6

This is a program for youth from Grades 1-6. They will find themselves engaged with hands-on projects, fun Bible stories, energetic games, and fun nights all the while under the care of great leaders.

I the parent give permission for my son to attend Boy's Club activities of the Community Christian Reformed Church of Richmond Hill, whether on the church property or away on an event. I also give permission to the leaders in charge to secure needed medical emergency treatment if needed. To ensure that all youth are safe here at Community Church, a Sign in – Sign out procedure was implemented in March of 2006. All youth members under the age of 16 are required to be signed in and out of our programs by their parents / guardians. If car-pooling occurs, parents/guardians may designate someone else to sign their child in or out of the program. A carpool driver may sign in their entire carload, and then sign them out again at the end of the evening. If names need to be added as car-pooling changes, please advise your leader or club coordinator. If, for some reason, you are not able to pick up your child, you may call Pastor Jack or the Club Coordinator with information on who is authorized to pick up your child, if not already on this form.*
